Which Chart Formations Are Unreliable

Ok...Now I'd like to know which chart formations have caused you to lose the most amount of money. If there is a trading method that has caused you to lose money over and over again, I'd like to hear about it... Give an example if possible.

In a book I read recently a scientific study was given to popular patterns, To my suprise the head and shoulders pattern had horrible results, only showing little significance in the forex market. Kind of funny how that pattern nailed this move in transports and the indicies
